HIGHER LEVEL HOOPS PARTICIPATION AGREEMENT, WAIVER OF LIABILITY, MEDICAL AUTHORIZATION & MEDIA RELEASE This agreement is entered into in consideration of the participant being allowed to take part in any Higher Level Hoops basketball camp, clinic, training, event, or related activity (collectively referred to as the “Program”). Higher Level Hoops operates as a ministry of: Free Indeed One Love Ministries — a registered 501(c)(3) Activities may take place at facilities including Hope Fieldhouse or other approved locations. The undersigned parent or legal guardian agrees as follows: 1. Assumption of Risk I acknowledge that participation in athletic and recreational activities involves inherent risks including, but not limited to: Slips, falls, and collisions Physical contact with other participants Equipment-related injury Overexertion, dehydration, or medical events Exposure to communicable illness Serious injury, paralysis, or death I understand these risks cannot be completely eliminated despite reasonable precautions and supervision. I knowingly and voluntarily assume all risks — known and unknown — associated with participation. 2. Health & Illness Acknowledgment I confirm the participant is physically capable of participation and will notify staff of any relevant medical conditions. I acknowledge that participation may involve exposure to communicable illness and agree not to hold Higher Level Hoops or associated parties responsible for illness or related outcomes. 3. Compliance and Conduct I agree the participant will: Follow coach and staff instruction Respect others and facilities Use equipment safely Remove themselves if unsafe conditions arise Program staff reserve the right to limit or terminate participation for unsafe or disruptive behavior. 4. Concussion & Injury Awareness I acknowledge basketball involves risk of head injury or concussion. I agree to: Remove participant from activity if injury suspected Seek medical evaluation if advised Follow return-to-play guidance if required 5. Comprehensive Release of Liability To the fullest extent permitted by law, I release and hold harmless: Higher Level Hoops (HLH) Free Indeed One Love Ministries (FIOLM) Hope Fieldhouse (HF) All coaches (paid or volunteer) Trainers, assistants, mentors, and team leaders Event organizers and program staff Volunteers of any kind Interns and student helpers Officers, directors, employees, and agents Sponsors or partner organizations Facility owners, operators, and lessors (collectively, the “Released Parties”) from any claims, liabilities, damages, losses, or expenses arising from participation, including those resulting from ordinary negligence except where prohibited by law. This includes claims involving: Injury Illness Disability Death Property damage 6. Covenant Not to Sue I agree not to initiate legal action against the Released Parties for claims connected to Program participation. 7. Emergency Medical Authorization I authorize Program staff to obtain emergency medical care or transportation if deemed necessary. Parents/guardians are responsible for maintaining their own medical insurance coverage and for all costs related to injury or illness sustained during participation in the Program. I accept responsibility for treatment expenses and release the Released Parties from liability related to medical decisions made in good faith. 8. Media & Promotional Release I grant permission for photographs, video, or audio recordings of the participant to be used by Higher Level Hoops / Free Indeed One Love Ministries for: Marketing Social media Website content Promotional materials without compensation or additional approval. 9. Registration & Payment All camp fees are non-refundable once payment is submitted. Registration is final. If Higher Level Hoops cancels a camp, participants will receive a full refund or transfer to another session. No refunds will be issued for missed sessions, scheduling conflicts, illness, injury, or withdrawal for any reason. 10. Personal Property Responsibility Higher Level Hoops and associated facilities are not responsible for lost, stolen, or damaged personal items. 11. Facility & Equipment Care Participants must respect facilities and equipment. Parents/guardians may be financially responsible for damage caused by intentional misuse. 12. Program Modification Higher Level Hoops reserves the right to modify scheduling, programming, staffing, or structure as necessary for safety or operational needs. 13. Severability If any portion of this agreement is deemed unenforceable, remaining provisions remain valid and enforceable. 14. Background Screening Statement Higher Level Hoops seeks to provide a safe environment and may utilize background screening procedures for staff and volunteers when appropriate. 15. Transportation Disclaimer Higher Level Hoops does not provide transportation unless expressly stated in writing. Parents/guardians assume responsibility for participant transportation to and from activities. 16. Injury Reporting Injuries should be reported immediately to staff. Documentation may be completed for safety and organizational purposes. 17. Christian Values Alignment Higher Level Hoops operates as a ministry promoting: Character Respect Encouragement Positive sportsmanship Participants are expected to uphold conduct consistent with these values. 18.
